The Lake Shore Athletic office would like to announce a new policy regarding how our athletes can get physicals to participate in sports.
After much consideration, the district will now allow athletes to go to their own physician for their physical. Each athlete, must obtain a form from either the school nurse or the athletic office to take to their primary care physician to be filled out. This form must be used by our athlete and then be returned to either the nurse or the athletic office. The form must be turned in prior to the first day of try-outs. If it is not then the athlete will not be able to participate until the completed form is returned to us. The district will still provide athletes an opportunity to see the school physician if they cannot see their own primary care physician. The dates are yet to be determined.
The athlete only needs to have one physical per school year. If they participate in a sport during any other of our three seasons they only need to bring in an update form.
